If your lifestyle questionnaire is not completed by 3pm the day prior to your appointment, it will be rescheduled as this does not give Dr. Twyman enough time to review your information and create a detailed plan moving forward.
Cassidy will be out of the office from Wednesday May 1, 2024 returning Friday May 5, 2024. We appreciate your patience with us in getting back to you.
